Apparel Order Form

This order form generates an email to those tracking the orders. We do not accept online payments, nor do we have real-time inventory tracking (some sizes may be out of stock). It does, however, give us a nicely formatted method to get you the apparel you desire.

You will receive two emails: the first is an automated email showing the information you typed in above. It should come within minutes after pressing Send. The second is one from a human being confirming your order, with further information, such as where to pick up, and so forth.

We do not ship the t-shirts; you (or your band student) can pick them up at school or band events. Payment must be made when you pick up the shirts; cash or checks made to Dennis Perry are accepted. You cannot debit these items from your band account.
